Let us help you design a home healthcare plan specific to your needs.

Step 1
Contact Spectrum
Connect with us via our website or telephone at (609) 614-0676. Our Customer Service Experts are available live from 8:00 AM EST to 5:00 PM EST Monday through Friday. If you leave a message after hours or if a Customer Service Expert is busy with another client, our goal is to contact you within 30 minutes from the time you left a message. If by web, we will reach out to schedule a call as quickly as possible.

Phone Intake & Needs Review
We verify your information, discuss your situation, and identify the type of support you or your loved one needs.
Step 3
Schedule Complimentary In-Home Assessment
Our Supervisor will schedules a complimentary in‑home assessment at a time convenient for you.
Step 4
In‑Home Assessment & Plan of Care Creation
We meet face‑to‑face to learn about your routines, preferences, safety needs, and daily challenges. We then create a personalized Plan of Care outlining tasks, schedule, and goals.
Step 5
Caregiver Match & Start of Services
We match you with a trained caregiver based on skills and compatibility, review the Plan of Care together, and begin services.

Our Quality Assurance Plan of Care
Follow-up Process

Step 1
Initial Follow-Up
After Services Begin

Within the first 7 days of service, the Supervisor contacts the client/family to confirm:

  1. Caregiver arrival and professionalism

  2. Tasks are being completed according to the Plan of Care

  3. No safety concerns or unmet needs

  4. Any adjustments needed to the schedule or tasks

Outcome: Early confirmation that services match expectations.

Step 2
Ongoing Supervisory
Visits

Supervisory visits occur:

  1. Every 60 days for personal care clients

  2. Every 90 days for companion-only clients

During each visit, the Supervisor:

  1. Reviews the Plan of Care with the client

  2. Observes caregiver performance (if present)

  3. Documents changes in condition

  4. Updates the Plan of Care if needed

Outcome: Continuous quality monitoring and compliance with NC DHSR.

Continuous Communication
& Plan Updates

Between supervisory visits, Spectrum maintains open communication through:

  1. Client/family check‑ins

  2. Caregiver reports

  3. Incident or change‑in‑condition alerts

  4. Adjustments to the Plan of Care as needed

Outcome: The Plan of Care stays accurate, relevant, and responsive to the client’s needs.
